RIAA provides a "certification report"

And if you like you can purchase a plaque from a licensed framer who works with you to determine a design.

Add: Previously I had a picture of a plaque that Scotty received (platnum) here that was removed by the moderator. The point I was illustrating was that one can create any design (subject to some approval) that one likes. In other words, there isn't an official plaque design, although current ones apparently all bear a "specialized RIAA hologram."
